The safety of our patients is of utmost concern at CHRISTUS St. John Hospital.
One area of patient safety is medication safety. When patients come into our Hospital, they bring with them a history of various medications that different doctors have prescribed for them. It is our responsibility as one of your health care providers to ensure we are aware of all medications you are taking, both prescribed by a doctor and those you take that are over-the-counter medicines.
When you are admitted to CHRISTUS St. John Hospital, you will be asked to provide a complete list of all the medications you are taking. Our staff will interview you about your medication history, and this card will help them obtain this information. They will investigate to ensure the accuracy and completeness of the information you provide to us. During the course of your hospitalization, if your doctors prescribe new medications for you to take or discontinue current medications, we will keep track of these and ensure they are provided to your next health care provider. In this way, you can rest assured that everyone who is taking care of you has the most current information about your medication history.
To help us provide the best care to you, we have developed a personal medication list that will allow you to keep track of the medicines you take. Whenever you visit your doctors or come to the hospital, bring this medication list with you to share with your health care provider and record changes to your medication.
