ImPact Baseline Test

Thank you for taking ImPACT. ImPACT is a computerized test specifically design to test for the effects of sports related concussions. As such you will be asked to answer several questions as well as take tests that measure your memory, concentration, reaction time, and how fast you can respond. This will take approximately 30 minutes to complete.
All of the information you are about to give are strictly confidential and in no way will it be released to anyone without your approval. Your test results will be reviewed for accuracy but personally identifiable information will never be disclosed with our your knowledge or consent.
You can complete ImPACT testing either as a baseline (before) or after you have already had a concussion. The baseline assessment is designed to determine where you are presently when you are in your typical state of health. The baseline assessment is not designed to determine if there is anything wrong with you before you may have had a concussion. Therefore baseline testing is not used as a medical test per se. Your performance during baseline testing is simply used to compare in case you have a sports concussion. In this case your ImPACT testing after the concussion will be compared to your baseline testing.
Completing baseline testing does not constitute a doctor-patient relationship with any member of the CHRISTUS St. John Concussion Center. It does imply, explicitly or implicitly our knowledge about your condition or that any member of CHRISTUS St. John Concussion Center is treating any medical condition for you.
ImPACT is a medical tool and should be treated as such. Any ImPACT test results should only be interpreted by qualified health care professionals such as a neuropsychologist or your physician. Any use by a non-qualified individual could lead to misinterpretation and as such could yield an incorrect diagnosis or treatment (or non-treatment) and lead to future negative consequences.
If you are taking ImPACT after you have been diagnosed with a concussion it should be done with the knowledge of your treating neuropsychologist, physician or athletic trainer or if you prefer CHRISTUS St. John Concussion Center. Then and only then should the results from your baseline and post-concussion testing be sent to your treater for appropriate interpretation and diagnosis and treatment recommendations.
No medical test is 100% accurate and as such ImPACT is designed to be used in conjunction with other medical findings/facts, records, and test results. It should be used as part of an overall diagnosis/treatment program for sports concussions. ImPACT test results alone are not sufficient to either rule in or rule out a concussion.
You are entitled to have a copy of your ImPACT test results. However, you should not try to interpret these findings. You have a right to have a copy of your records and they should be treated the same way as any other medical records you have keep for personal records.
